OUR MODEL FOCUSES ON EMPOWERMENT

When services are provided in the home there is a very unique ability to actually practice the target skills throughout the family’s environment and this includes teaching how to advocate and communicate for themselves, how to access resources within their home environment. By treating the client and family in the home environment it has a long term impact, sustains change, moving away from treatment dependency or out of home placement or higher levels of care.

INTENSIVE IN-HOME SERVICES

Crisis Level I:

  • High level of care without immediate intervention
  • 2-4 in-home visits per week
  • Crisis intervention
  • Emphasis on the family system
  • Relationship, communication and in-home structure skills
  • How to effectively stabilize a crisis
  • Coordinating care providers

Crisis Level II:

  • Less intensive with no immediate crisis present
  • 1 in-home visit per week
  • Preventative skills to decrease escalations
  • Increase family stability and resourcefulness
  • Parenting skills and development
